www.airflo.com has all the stuff to make one, you can buy it pre-built from them or go to your local exhaust shop and get the tubing and order the napa filter from FleetFilter.com
I wouldn't put it into the exhaust. Most places in the exhaust will have more pressure than the CCV will have. If you had to put it in the exhaust you'll want to do it in a place where the pipe changes sizes and then I'd check to make sure what is ACTUALLY happening there with a manometer.
I'd just leave it open to the atmosphere or if you want to build a catch can to pull the oil out of vapor.
You might not loose a boot but that oil coating the inside of the CAC isn't helping the heat transfer effiency. I have a road draft tube, simple and it works.
