Might be stupid questions, but on my livewire I've always ran:

Extreme, stock trans, 34.5" tires, egr not disabled, aftermarket intake.

With that setup I personally got best mileage. Would anything change if I disabled egr?

Also, if on Trans Only tune I chose all same options so does that mean It's stock power but able to run dpf delete?

On the base tunes, the option turns off erg related codes. On our custom programs it will also disable the EGR system for customers who have installed a delete kit.

they are canned tunes.
i have never gotten a code of any sort.

i did confirm to myself today that trans only is just what i thought, a tune to run DPF delete with no HP added.

if i run my extreme with all my said above settings but EGR delete disabled (even tho all my EGR is there) what will happen?

You can run the EGR disable or enabled option. It won't make a difference at all since your EGR is intact.
