2006 6.0 started missing and blowing black smoke.

So i was driving and the truck started to shudder and miss all of the sudden and lost power and was making black smoke. I shut it off right away because i didnt want to damage anything and got it towed home. Plugged in my sct TS and found a PO264 code Injector 2 circut low. Just had the heads off 6000 miles ago for lifters and is it possible a injector could have loosend up? Any help would be greatly appreciated! Thanks.
