Internal ZF5 issue?

When I drive the truck and shift gears, say 3rd to 4th, when I release the clutch pedal the trans makes a Bang/clang sound (sounds like 2 metal plates slapping flat against eachother). When I back the truck up an incline into my driveway if I don't slip the SH*T out of the clutch it sounds like the same noise but it is horrible and the truck bucks and drivetrain bangs back and forth. I am not knowledgeable in transmissions just wondering if anyone has any thoughts? it is a 300k trans (can't buy anything good anymore I tell ya :-) )
