IPR questions

Whats all the possible symptoms of a failing IPR valve or o ring? were is it located and what needs to be bought to fix it

1986-FORD F-250, 4X4, T-19,6.9 bored.020 MANY MODS, and broke again

1995-F-350, ext cab, DRW, 2WD, 291,xxx miles , ZF5, MODS:chopped exhuast, FPR shimmed, 6637, gutted ebpv
