HPOP failing 7.3?

95, 7.3 powerstroke, 248k, 4EOD, baby swamps with their tunes on a ts6 chip, 4.10.

Ever since I put new injectors (baby swamps) in about 15,000k ago and I am pulling a grade with a trailer on, I am throwing a CEL.
I have forescan and it says I am making around 2696-2800 or so. The numbers dont sound off to me, but they are not steady. While driving and cruising at 70mph, the numbers bounce around alot. Like 200 or so psi within a second. I don't know if this is a sensor/forescan or if it is normal. But when ever i try and read the code, it kills the truck(never done it before with forescan) and the light is gone when I restart the truck.

But every time I would decide to put my foot in it going from 70 to 80(2,200rpm-2500rpm), the HPOP would drop down to 2500-2400.
Any idea on what is going on? MY IPC is sitting at around 40%-42%. When I would put my foot in it, I would hit around 15psi. It can't be an overboot code because I have ran it all the way up to 25psi on the same towing tune and never threw a CEL.

The truck runs like nothing is different when the light is on, Im stumped. New ICP a year ago...
Any ideas?

95 Auto. 4.10 gears. SCLB. Stock besides Baby Swamps Injectors and TS-6 chip with swamps tunes. AFE MaxFlow. Glowshift trans, pyro, and boost. 6.0 IC KC TP38R. Converted EBPV to exhaust brake.
