What went wrong?

I am looking for suggestions on what happened here. Had a new set of d-Tec injectors put in. About a month later the truck started to run away pulling a load. It finally just #### itself down. Got it towed back to shop and found the crankcase was full of diesel. Pic of first cylinder on driver side.

I think DIPACO DTech needs to answer that!

Can you verify that the torquing procedure was correct/accurate on the hold-downs and that new copper washers were used?
